BLHS Athletic Clearance Information
The Sumner-Bonney Lake School District uses an online athletic clearance process that allows families to complete most requirements electronically through FinalForms.
Create or access your FinalForms account at sumner-wa.finalforms.com.
Online registration and athletic clearances are available in the weeks before each sports season (fall, winter and spring).
When registering through FinalForms, the system tracks each athlete's clearance status.
Athletic Clearance Steps
- Complete all required forms in FinalForms (both the student and parent/guardian must complete their sections).
- Purchase a 2026-27 ASB card through the bookkeeper beginning Aug. 17 or online beginning Aug. 1.
- Pay the athletic participation fee through the bookkeeper or online beginning one month before the sport season.
- Clear all school district fines with the bookkeeper.
- Have a current athletic physical on file with BLHS. New or updated physicals may be submitted to Mr. Ferguson in the Student Services Office beginning Aug. 17 or uploaded to FinalForms at any time.
- Pick up an athletic clearance ticket from Mr. Ferguson in the Student Services Office during the week before your sport begins.
Coaches will be notified of all cleared athletes before the first practice.
Athletes must be cleared before participating in any practice or competition.
